It may sound weird, but the bug didn't impact boss spawning items. It was only cosmolight, and both tears. If using Fargo's mutant mod it also happened with any event summoning items from that mod. Not bosses though. If you are having problems using boss items it's likely another issue, or possibly a connection issue.
